Submitting a Comment or Complaint

Our aim is to provide the highest level of care for all our patients.  However, if you have discovered or encountered a problem with the practice or the care provided, we are happy to listen to any comments or complaints as to how we can improve the practice going forward.  If you do have a comment or a complaint, please speak to any member of staff intially with your complaint. This gives you the opportunity to resolve any concerns you may have without it going through a formal process. You can put your complaint in writing or complete one of our forms either online, or by printing off and dropping it off at the surgery reception. 

If it is a complaint you wish to make, please ensure the complaint is enclosed within an envelope and addressed to the Practice Manager, Nicola Levey, to ensure privacy or you can email n.levey@nhs.net.

A copy of our complaints procedure is available from reception and is also displayed on the wall in the waiting room.

Medical matters cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.
