Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode.
You will need to complete a Request for Acceptance Application Form/Health Questionnaire, Please pick up a form from reception or download and print form from this website. 
Please ensure you have completed the form correctly & signed the Patient-Practice Agreement on second page.  Complete a child application form for each child in your care.

Application Form - Child (under 16 yrs)

This is NOT a registration form.  Your application forms will be passed to the doctor, who then decides whether to accept your application. Please wait approximately 1 week to find out if you've been accepted.  
Once accepted then you will need to complete the Registration form (GMS 1) which you can pick up from Reception.  Please bring along proof of address such as a recent utility bill/driving licence/passport
All new patients are offered a health check with a member of the healthcare team. Medical treatment is available from the date of registration.  Please contact reception for further information.
